Please Help Design this form

Hi there. I am trying to design an online form and am having a hard time doin this. Not very good with html and javascript. I need a 3 step form.

1st Page - Name
Email
Contact No.
Address
Delivery Address

Continue

2nd Page - (Product Id: xxxxxx Product Location: xxxxxxxx qty:xxx Price: xxxxx) All in one line. plus option to add more lines if more products ordered. + sign

SubTotal - Qty x Price
Automatically add $75 service charge - Total

Continue

3rd Page - Drop down list with States (upon selecting state) Next drop down list with postcodes based on that state. Upon selection a price gets added to the total visible on the right hand side.

Tick box - agree to TOC. Upon clicking only then would pay now button get activated.

4th Page - 2 payment options given - 1 takes to paypal and the other open a small window showing bank details to make payment via bank transfer. For this method the form gets sent to client via email.

I know sound hard but please please guide me through creating this.
